I was born and raised in New Jersey. I’m married to a super achiever, my wife Dina since 1986. I’m a father to 3 handsome sons.
WORK EXPERIENCE
I worked in a dental clinic from 1987-88. However, since 1989 I have operated a private dental office, delivering quality care and improving oral health in Palm Harbor.
EDUCATION
I received a bachelor’s degree in biochemistry from Rutgers College. I attended UNC-Chapel Hill where I received my dental degree. I love attending continuing education courses about digital dentistry and implant technology.
FUN FACT
I love playing Christmas carols on the accordion and singing karaoke with friends, playing golf with my sons and traveling with my family.

ABOUT ME
I have been a resident of Palm Harbor, Florida since 1994. I am married with 2 amazing
daughters and 3 beautiful grand-daughters. I love
laughter.
WORK EXPERIENCE
I have been a Super Hero and Dental Assistant for 40 years, working with Dr. Kostakis for 26 years. Helping to preserve and improve smiles one appointment at a time.
EDUCATION
I attended Mount IDA College located in Newton, Massachussetts and earned my AS degree.
FUN FACT
I love camping and watching John Wayne movies.
Chaye Walker
ABOUT ME
I was born and raised in Atlanta, Georgia. I am a wife, mom, dog mom, and grandma. I became a Floridian in 2021.
WORK EXPERIENCE
I have been in dentistry for over 16 years. I started independent practice consulting 10 years ago. I am a published author and a Certified Life Coach.
EDUCATION
I earned my AA Degree from University of West Georgia, and I earned my MBA from Saint Leo University. I am currently enrolled in a Doctoral Program for Religious and Biblical Studies.
FUN FACT
I spent a year in Bali, Indonesia earning a certification through Yoga Alliance and learning the extraction process of essential oils and herbal medicine.
Andrea Ballenger
ABOUT ME
I was born in Salt Lake City, Utah and have lived in Pinellas County Florida for most of my life. I have been married for 27 years and have 1 daughter. I’m a dog mom to Queen Hazel whom my family and I rescued in 2021.
WORK EXPERIENCE
I have been working as a Front Desk Magician with Dr. Kostakis for 20 years. I have really enjoyed building and developing personal relations with the patients.
EDUCATION
I attended St. Petersburg College and earned a Dual AS degree in Marketing Management and Business Administration. I attended Central Florida Institute where I studied Dental Assisting.
FUN FACT
I love going to the beach and taking long walks.
Melissa Vasilakos RDH
ABOUT ME
Happily married for 31 years with two lovely daughters, my passion is educating and encouraging patients on importance of oral health.
WORK EXPERIENCE
Joined Dr. Kostakis’s team in June 2021, and I have over 15 years of experience in the dental field.
EDUCATION
I earned my degree in Dental Hygiene in 1994 from Pasco Hernando State College.
FUN FACT
I enjoy volunteering at the NICU Trinity Memorial Hospital and spending time with family and friends in sunny Florida.
Majlinda “Nina” Plyku RDH
ABOUT ME
I have lived in sunny Florida since 2006. I am married and have two handsome sons. I love having the confidence of knowing I deliver quality dental care and teaching the importance of overall oral health.
WORK EXPERIENCE
I have been working as a Dental Hygienist and changing lives for 20 years.
EDUCATION
I attended Cuyahoga Community College in Cleveland, Ohio and earned my AS degree in Dental Hygiene.
FUN FACT
I love jogging, yoga, traveling, and going to the beach.
